An Analysis of the Psychiatric Characteristics of the Repeater among Hospitalized Adolescents

Abstract
In this paper. the author tried to control repeater syndrome by elucidating typical clinical features of college entrance examination stress(CEES), comparing repeater students with senior high school students and college students. Subjects were adolescent patients including repeater students(N=115), senior high school students(N=91), college students(N=89) who were admitted in Phil-Dong and Yongsan Hospitals, Chung-Ang University Medical Center, from 1981 to 1995. Data were processed by IBM PC computer using SPSS/PC^+ program and were statistically analysed by chi-squre test and F-test. The results were as follows; 1. Comparing repeater students with the other groups, repeater students group showed a higher tendency in severe psychotic disorders with a serious psychopathology like paranoid and psychotic symptoms as well as paranoid, histrionic, hostile and narcissistic personality traits. Also majority of them regarded as anxious and confused subtype among climical subtypes of the CEES. 2. Repeater students group felt more stress about prvious school life and examination as a result of poor academic performance than the other groups. Particularly, due to stress concerning forthecoming college entrance examination, repeater students group were easily predisposed to CEES syndrome as compared to senior high school students. And also, repeater students group showed serious social maladaptation mainly due to serious psychosocial stressors in their school life or heterosexual relationship. 3. Repeater students group showed higher rates of long-term admission to closed ward involuntarily and were permitted frequent field trip while main therapeutic drugs were neuroleptics such as holoperidol and chlorpromazine. 4. Repeater students group showed resistances to the treatments such as outright rebelliousness, somatization, leveling pseudo-stupidity and over-submissive. These resistances could be resolved by supportive psychotherapy and physical restraint or seclusion method. The majority of parents(75.7%) of hospitalized adolescents showed resistances caused by pathologic symbiotic relationship between adolescent patients and their parents, therapist frequently used visitation and filed trip permission to cope with these resistances.
